First Visit

Your First Visit

When you visit, you'll immediately feel welcomed. We want to know about you and your goals for future health.

We will take a comprehensive medical and dental health history and ask you about your concerns and problems. If necessary, X-rays and photographs will be taken, as well.

Dr. Hamm will then meet and discuss your goals, concerns, and past dental experiences. Do you need replacement restorations, cosmetic enhancements, or relief from oral pain or headaches? By learning about your expectations, Dr. Hamm can develop a customized plan to acheive what you want, to meet your dental goals.

Thank you for choosing Dr. Hamm and our team as part of your healthcare team. We're very happy to have you in our patient family. Rest assured, we will always make every effort to meet and exceed your expectations for clinical excellence and great service.

Financial Policy

You may pay for your dental care by cash, personal check, Visa, MasterCard, American Express, or Discover. Please plan to make payment at your visit unless other arrangements have been made. Dr. Hamm is not in network with any insurance company and does not accept assignment of benefits therefore full payment will be required at time of treatment. We will file your insurance claim electronically to expedite your reimbursement of our fees, And work with you to maximize your benefits. Most insurance companies pay claims to patients within two weeks.
